HOW TO APPLY FOR THE ELECTRONIC POSTAL ID in Malaybalay city Bukidnon 2015

1. Duly accomplished application form (click to download)
(3 copies, all in CAPITAL LETTERS and use black pen only)
2. All applicants for the new Postal ID are required to submit any one of the following identification documents as proof of identity:
A. Original Birth Certificate issued by NSO
B. Original Birth Certificate issued by LCR
C. Form 1A issued by LCR within six months
D. Valid GSIS UMID card
E. Valid SSS UMID card
F. Valid passport
G. Valid LTO Driver's License
3. Barangay Clearance - (for Postal ID Purpose or Any Legal Purpose)

4. Additional: FOR MARRIED FEMALE APPLICANTS
Original MARRIAGE CERTIFICATE issued by the NSO or Local Civil Registrar - FORM 3A
5. Pay the EPID(Electronic Postal ID) fee P414.40

6. DATA CAPTURE AT VALENCIA CITY POST OFFICE
Bring the Official Receipt(OR) (Moadto sa Valencia city Post Office-in front of Legislative building in the municipality of  Valencia)

POSTAL ID CAN BE CLAIMED AFTER 3 WEEKS TO 1 MONTH 
AT MALAYBALAY CITY POST OFFICE
